How to Get Started

Fremont Hospital is dedicated to providing immediate help to you and your loved one. With a call to Fremont, you have immediate access to a team of multidisciplinary professionals 24 hours a day, seven days a week.

Professional Referrals

We welcome referrals from emergency rooms, hospitals, clinics, psychiatrists, mental health professionals, social service agencies, school systems and insurance companies. We evaluate potential patients for admission to learn which treatment option—inpatient, partial or outpatient—is most appropriate to the individual’s needs.

Schedule an Assessment

Fremont Hospital is dedicated to providing immediate help to you and/or your loved one. With a call to Fremont, you have immediate access to a team of multidisciplinary professionals 24 hours a day, seven days a week. These mental health professionals assess an individual in crisis and arrange admission to the appropriate level of care.

What Happens During the Assessment

Behavioral health care starts with an evaluation comprised of a clinical interview with one of our mental health specialists. Evaluations may last from 1-2 hours, as you will meet with a masters-level clinician or nurse. During this time, a member of our professional staff meets with you to evaluate your needs and review your treatment options with you.

What to Bring to Your Scheduled Assessment Appointment

  • Identification card
  • Insurance card
  • Pharmacy card
  • Guardianship documents, if applicable
  • Custody documents for children and adolescents, if applicable

What Happens at Admission

During the admissions process, you will meet with an intake specialist who will review your rights, insurance information and have you sign various consents for treatment. You will also have a series of comprehensive evaluations, including a nursing assessment, psychosocial evaluation, oral history and a physical exam.
